Community HEARTS Screening Initiative Hypertension Early Awareness and Reduction 

The Partnership to Advance Cardiovascular Health invites community organizations, health centers and partners to apply to participate in a nationwide hypertension screening and awareness initiative launching during Hypertension Awareness Month.  

This initiative aims to provide microgrants to individuals and organizations seeking to expand access to blood pressure screenings, improve early detection of hypertension and connect individuals—particularly those in underserved communities—to education and follow-up care. Selected sites will receive support for screening implementation, outreach and educational resources.
